Contest: Win a 5-Pack of OWR Corojo Toro

I want to give away some free cigars and want to test a new form. I thought I would combine the two and run a contest. I have a 5-pack of OWR Corojo toro’s up for grabs. Pretty simple, just visit the Submit A Cigar Review page and fill in the blanks. Any feedback on the form would be greatly taken (to long, bad layout, etc).
